Hair loss: the basics

Options to discuss with your doctor

This is not medical advice, just handy to know. Talk to a doctor if you want to explore this option further.

  • Topical minoxidil 5% (men's pattern hair loss)

    Over the counter

    What studies found

In a 48-week trial of 393 men with pattern hair loss, 5% minoxidil solution applied twice daily beat 2% and placebo, with about 45% more regrowth than 2% at week 48 and faster onset.

In a 48-week trial of 393 men with pattern hair loss, 5% minoxidil solution applied twice daily beat 2% and placebo, with about 45% more regrowth than 2% at week 48 and faster onset. Itch and scalp irritation were more common with 5%. Stopping it ends the benefit. The label covers the top of the scalp, not a receding front hairline, and results take months.

Dose as studied: 5% solution or foam applied to the scalp, twice daily in the pivotal men's trial

Status: Sold over the counter in the US, UK and most of the EU for adults, with labels that differ by sex and strength. Read the label, and ask a doctor or pharmacist if you have heart or blood pressure problems.

  • Oral low-dose minoxidil (prescription, used off-label)

    Prescription only

    What studies found

    • In 90 men over 24 weeks, oral 5 mg and topical 5% did not differ significantly in hair density
    • Pooled analysis of 279 people: no density difference between oral and topical (difference 0.02)

Side effects: Extra body or facial hair in 15.1%; lightheadedness 1.7%, fluid retention 1.3%, fast heartbeat 0.9%; 1.7% stopped; no life-threatening events.

Read more why it works and the research

In a 24-week trial of 90 men, oral 5 mg and topical 5% minoxidil did not differ significantly in terminal hair density, though oral looked better on vertex photographs, and hypertrichosis (extra body or facial hair) affected 49% on oral. A pooled analysis of 279 people found no density difference (standardized difference 0.02) and about double the hypertrichosis risk (relative risk 2.01). In a cohort of 1,404 people, hypertrichosis was 15.1%, lightheadedness 1.7%, fluid retention 1.3% and fast heartbeat 0.9%; 1.7% stopped for side effects, and no life-threatening events occurred.

Dose as studied: 0.625-5 mg/day studied; 1.25 mg/day common in trials

Status: Prescription only. Minoxidil tablets are approved for high blood pressure, and use for hair loss is off-label. It is not for use in pregnancy or when trying to conceive, or while breastfeeding, unless a doctor advises it. Discuss with a doctor, who will check your blood pressure and heart history first.

  • Clascoterone 5% scalp solution (men, not yet approved)

    Not approved yet

    What studies found

    • In two phase 3 trials (1,465 men), the 5% solution beat vehicle on hair count at 6 months in both
    • Men who stayed on it to month 12 had 2.39 times the improvement of men switched to vehicle
    • Company-reported, not peer-reviewed; absolute hairs per cm2 gained are not published

Side effects: Side effects were similar to the vehicle solution in the phase 3 trials.

Read more why it works and the research

In two phase 3 trials of 1,465 men, a 5% clascoterone solution on the scalp beat the vehicle solution on hair count after 6 months in both trials, with side effects similar to vehicle. Men who kept using it to month 12 had 2.39 times the hair count improvement of men switched to vehicle at month 6, and counts fell after switching. Clascoterone blocks the androgen receptor in the skin itself and is designed to break down once absorbed. These results are company-reported and not yet peer-reviewed, and the absolute hairs per cm2 gained have not been published, so it cannot yet be compared head to head with finasteride or minoxidil.

Dose as studied: 5% solution on the scalp, as studied in the phase 3 trials

Status: Investigational for hair: not approved in the US, EU or UK, and no hair product is for sale. The 1% acne cream (Winlevi) is a different product. Women were not studied. Discuss with a doctor.

The short answer

Most adult hair loss is slow pattern loss (gradual thinning in a typical pattern), and some medicines have trial results behind them. This is not medical advice, just handy to know. Talk to a doctor if you want to explore this option further.

See how it works

Each follicle runs its own clock, so you never shed everything at once. About 85 to 91% of scalp hairs are growing at any moment and roughly 9 to 15% are resting. Natarelli 2023 ↗ Rebora 2002 ↗
Simplified diagram of the Norwood scale. Some temple recession after puberty is almost universal, and even dermatologists agree only moderately on stage. Treat a stage as a rough band, not a diagnosis. Norwood 1975 ↗ Gan 2005 ↗ Guarrera 2009 ↗
Simplified diagram of part-width scales used for women. Women usually lose hair as diffuse thinning with a widening part and the front hairline kept, rather than male-type recession. Ludwig 1977 ↗ Ramos 2015 ↗

Do this

  1. 1Photograph your hairline and crown every 3 months, same light.
  2. 2Shedding 50-150 hairs a day is normal; thinning you can see is not.
  3. 3Stop tight styles that pull on the hairline.
  4. 4Skip biotin and 'detox' products unless a doctor finds a deficiency.
  5. 5Medicines in trials take months to show results, and gains fade if stopped.
  6. 6Talk to a doctor early, and fast for patches, sudden shedding or scalp pain.

The numbers behind it

  • 50%

Of men are affected by pattern hair loss by age 50 (genetics study of about 70,000 men).

  • 26.3%

Hair density lost over 5 years by men on placebo (239 hairs in the test area). 75% looked worse on photos.

  • 45% more

Regrowth with 5% minoxidil lotion (a medicine) was 45% higher than with 2% after 48 weeks, in 393 men.

  • 107 hairs

Extra hairs versus placebo after 1 year on finasteride, a prescription pill (1,553 men; counted in a 5.1 cm2 crown patch).

For your hair

  • Men (Norwood scale)

The hairline recedes at the temples, then the crown thins. Stages run I to VII.

  • Women (Ludwig scale)

The part widens and thinning spreads over the crown. The front hairline usually stays.

  • Sudden or patchy loss

Shedding over weeks, round patches or a sore scalp are not pattern loss. See a doctor.

Myths to ignore

  • Myth A receding temple means you are going bald.

Truth Some temple recession is near-universal in adults: 98.6% of men and 64.4% of women in one Australian town survey, and none of 140 children.

    Gan 2005 ↗
  • Myth Washing your hair often makes it fall out.

Truth The drain hairs were already shed: about 28 per wash in people without hair loss, 52 with pattern loss. Pattern loss itself comes from hairs thinning.

    Li 2022 ↗
  • Myth Biotin and hair vitamins regrow hair.

Truth A 2024 systematic review found no high-quality support for biotin as a hair supplement. A true deficiency is a different matter; ask a doctor.

    Yelich 2024 ↗
  • Myth Cosmetic serums work like medicines.

Truth Adenosine, the best studied, was positive in all 7 trials, but certainty ranges from very low to moderate. Pooled lotion results for density were not significant.
