SCALP
Dandruff shampoo and thinning hair
In short
Ketoconazole shampoo improved hair density almost as much as minoxidil in one small trial. The best-proven hair-loss options are minoxidil and anti-DHT drugs such as finasteride.

What you need
Do this
-
1
Thinning plus flakes? Treat both; the scalp yeast may inflame the follicles.
-
2
Trial used 2% ketoconazole shampoo, off-label for hair. Why
Talk to a doctor.
-
3
Wash 2 to 3 times a week, leave on for the label time.
-
4
Do not take breaks: inflammation markers rose within 3 days of stopping.
-
5
What the science points to: other antifungal shampoos might too (possible).
-
6
Dandruff care adds to the proven options, it does not replace them.
The numbers behind it
-
Almost equal
Hair density, size and share of growing hairs improved almost equally with 2% ketoconazole shampoo and 2% minoxidil in pattern hair loss.
-
3 days
How soon histamine and complement C3 rose after switching to a plain shampoo. Flaking rose after about 3 weeks (industry study).
-
318 people
In 5 human studies of topical ketoconazole for pattern hair loss, hair shafts got thicker. No large randomized trial exists yet.
For your hair
Other antifungals (same yeast target)
Piroctone olamine, selenium sulfide and ciclopirox act on the same yeast. What the science points to: a similar effect is possible; the trial used ketoconazole.
EU shoppers
Zinc pyrithione has been banned in EU cosmetics since 1 March 2022. Ketoconazole, piroctone olamine and selenium sulfide remain.
Women with diffuse thinning
The trial looked at pattern hair loss. Dandruff care is worth doing for flakes or itch. A doctor can look at the cause of thinning.
Myths to ignore
-
Myth Dandruff shampoo makes hair fall out.
Truth In the trial, ketoconazole shampoo improved hair density.
Piérard-Franchimont 1998 ↗ -
Myth You can stop once your scalp looks clear.
Truth Scalp markers worsened after about 3 days on a plain shampoo, before any flakes showed.
Locker 2025 ↗ -
Myth Ketoconazole shampoo is a proven hair-loss treatment.
Truth The best-proven options are minoxidil and anti-DHT treatments. Ketoconazole results come from small studies, and no large randomized trial exists.

Go deeper
- DandruffDandruff is a common, chronic flaking of the scalp driven by yeast, skin oil and your skin's sensitivity, and an antifungal shampoo controls it well as long as you keep using it.
- Seborrheic dermatitisSeborrheic dermatitis is dandruff with redness and inflammation, and it is controlled rather than cured with antifungal shampoos and, for flares, short-term anti-inflammatory treatment.
- Androgenetic alopecia (male and female pattern hair loss)The most common cause of hair loss is an inherited, slowly progressive shrinking of scalp follicles; it cannot be cured, but finasteride, minoxidil and transplants slow, partly reverse or camouflage it.
- Scalp health and oxidized sebumSebum, scalp microbes and inflammation clearly drive dandruff; the claim that scalp buildup or oxidized sebum causes hair loss is not supported by human data.
- When to see a dermatologistMost gradual thinning is pattern loss you can start treating yourself, but scarring signs, scalp symptoms, patches or sudden shedding should go to a clinician quickly because delay can cost permanent hair.

Piérard-Franchimont C et al. Ketoconazole shampoo: effect of long-term use in androgenic alopecia. Dermatology. 1998
Tested: 2% ketoconazole shampoo versus unmedicated shampoo, with or without 2% minoxidil
Result: Hair density, hair size and the proportion of growing hairs improved about as much with ketoconazole as with minoxidil; sebum fell.

Result: Human studies showed increased hair-shaft diameter and pilary index.

Result: Biomarker deterioration (Malassezia load, barrier disruption, inflammatory and oxidative markers) appeared after about 3 days and visible flaking after about 3 weeks. Randomized double-blind; industry study.

Tested: 2% ketoconazole shampoo twice weekly, then weekly or alternate-week prevention, versus placebo
Result: 88% had an excellent response; relapse within 6 months was 19% with weekly ketoconazole versus 47% with placebo.

Result: Zinc pyrithione is classified as toxic for reproduction (CMR 1B). The SCCS judged it safe as a rinse-off anti-dandruff up to 1%, but as no lack of alternatives was shown it was moved to the list of banned substances (Annex II), applying from 1 March 2022.
