Topic

Smoking, alcohol and hair

Smoking goes with early graying and probably earlier pattern loss, but the evidence is observational and mixed.

Key takeaways

  • Smoking is associated with early graying and probably earlier pattern hair loss, but the evidence is observational. A 2013 cross-sectional study of 207 people found smokers were about 2.5 times more likely to gray before 30 (adjusted OR 2.5; 95% CI 1.5-4.6). Zayed 2013 A 2024 scoping review of 65 studies lists cigarette smoking among reported risk factors for early-onset pattern hair loss, alongside family history and high BMI. Liu 2024
  • Not every study agrees. In 256 Iranian men with pattern loss versus 256 controls, smoking, hookah and alcohol use did not differ between groups. Danesh-Shakiba 2020
  • Alcohol evidence is thin. A 2026 systematic review of 17 diet-hair studies found higher alcohol and sugary-drink intake correlated with more hair loss. Gomes 2026 In a Turkish graying survey, alcohol use was more common in premature graying. Akin Belli 2016 Heavy drinking can also harm hair indirectly through poor nutrition (zinc, folate, protein).
  • Smoking, alcohol, sleep and obesity are discussed as lifestyle factors in alopecia areata, with limited and mixed evidence. Minokawa 2022
  • Quitting is worth it anyway for general health. A hair benefit is plausible, through oxidative stress and blood supply, but untested.
  • We found no hair-specific human data on vaping or cannabis.

The science

Smoke contains oxidants and toxins that may damage pigment cells and follicles; the graying authors link premature graying to oxidative stress. Akin Belli 2016 Pattern hair loss is highly heritable, with more than 380 associated genetic loci, so lifestyle probably shifts timing and severity rather than creating the condition. Liu 2025

What it means by hair profile

  • Family history of early graying or loss: genes are the main driver; lifestyle is an add-on.
  • Women and non-smokers: most studies are in men, and data for women are sparse.
  • Dry, brittle hair in heavy drinkers: check zinc, folate and protein intake (diet and hair).

Myths and facts

  • Smoking directly causes baldness: associated, not proven.
  • A glass of wine hurts hair: no evidence at light intake.
  • You can reverse graying by quitting: not shown.

Practical recommendations

  • If you smoke, quitting helps general health and possibly scalp circulation; do not expect hair regrowth.
  • Keep alcohol moderate and eat adequately.
  • Use proven treatments for pattern loss rather than relying on lifestyle alone (androgenetic alopecia).

Bottom line

The link is real in some studies and absent in others. Quit smoking for your health, and do not expect it to fix hair. Discuss with a doctor.
