Topiramate
An epilepsy and migraine pill sometimes floated as a topical hair grower, although nobody has ever tested it on scalp hair; the real hair signals are reversible shedding, one case of overlong eyelashes and a small open trial in compulsive hair-pulling.
What it is
Topiramate is a prescription pill for epilepsy and migraine prevention, and in the US one half of a weight-loss combination (phentermine/topiramate). It is cheap and generic. No topical product exists. The only topical work is a 2% cream made for mouse wound experiments.
How it works
The US label says topiramate blocks sodium channels, boosts GABA, blocks AMPA/kainate glutamate receptors and inhibits carbonic anhydrase. None of these is a known hair-growth pathway. Two loose threads link it to hair:
- A 1974 report described excess body hair (hirsutism) after long-term acetazolamide, the classic carbonic anhydrase inhibitor, and a 2005 letter described abnormally long eyelashes with topiramate. Two reports in 50 years are a curiosity, not a mechanism. Weiss 1974
- In mice, a 2% topiramate cream on skin wounds did not speed closure but left thicker repaired skin with more of the stem-cell factor SOX2. Hair growth was never measured. Jara 2018
What the studies show
- No published study of topical topiramate on hair growth exists in humans or animals, and no trial is registered for hair loss (checked 2026-10-10).
- One case report attributes acquired eyelash overgrowth (trichomegaly) to oral topiramate; the full text was not available to us. Santmyire-Rosenberger 2005
- In an open-label pilot of 14 people with trichotillomania (compulsive hair-pulling), 6 of 9 completers responded over 16 weeks. There was no placebo and 5 dropped out with side effects. Lochner 2006
- A review of 1,656 reported antiseizure-drug hair-loss cases counted only 13 for topiramate, against 983 for valproate. The loss was diffuse and reversible. Pitton Rissardo 2023
- A single patient on several antiseizure drugs shed diffusely about 2 weeks after adding topiramate and recovered within 2 weeks of stopping. Unler 2025
- The US label reports alopecia in 3% (50 mg/day) and 4% (400 mg/day) of adults in a monotherapy trial with no placebo arm. Topamax label
- In diabetic mice, the same 2% cream improved wound closure (98% vs 81% at day 10). Mouse skin wounds only. Jara 2019
How it's used in studies
Oral 50-250 mg/day (flexible dosing for 16 weeks) in the trichotillomania pilot. In mice, 20 microlitres of 2% cream once daily for 14 days on wounds. No human topical dose has ever been studied.
Side effects and safety
- Oral topiramate can cause diffuse shedding and, rarely, alopecia areata in case reports.
- Pregnancy risk is serious: higher risk of birth defects and low birth weight, and a UK regulator review cited roughly 2-3 times the risk of intellectual disability, autism and ADHD after exposure in the womb. The UK has required a Pregnancy Prevention Programme since 20 June 2024. MHRA 2024
- Other label risks include acute glaucoma, metabolic acidosis, kidney stones, reduced sweating, mood effects and severe skin reactions.
- Topical use is unstudied; absorption from a homemade preparation is unpredictable, so systemic effects cannot be ruled out.
Bottom line
There is no evidence that topical topiramate grows scalp hair, and the drug carries serious pregnancy and body-wide risks. Proven options are minoxidil and finasteride. Discuss with a doctor.
Fun facts
- A drug better known for causing shedding has one published case of making eyelashes grow abnormally long.
- The first carbonic anhydrase inhibitor, acetazolamide, was reported to cause excess body hair in 1974, three decades before the topiramate eyelash report. Weiss 1974
