Dipyridamole

An old antiplatelet pill that on paper boosts the same adenosine pathway topical adenosine uses, but no study has ever tested it on hair and its label lists hair loss as a rare report.

Prescription medicine, used off-label for hair Cost varies Safety: moderate

hair lossthinning

What it is

A prescription antiplatelet and vasodilator (US, EU, UK), sold as tablets, as a capsule combined with aspirin for stroke prevention, and as an injection for cardiac stress imaging. It is generic and inexpensive. There is no topical hair product.

How it works

Dipyridamole blocks adenosine uptake into cells, raising local adenosine, and inhibits phosphodiesterases, which amplifies nitric-oxide signalling. The hair rationale is borrowed from adenosine:

  • In cultured human dermal papilla cells, minoxidil's effects were blocked by adenosine-receptor blockers, suggesting minoxidil works partly through adenosine. Li 2001
  • Adenosine raised the growth factor FGF-7 more than two-fold in those cells, and FGF-7 lengthened human hair shafts in organ culture. Iino 2007
  • A rare genetic disease, H syndrome, caused by loss of the nucleoside transporter ENT3, includes excess body hair and darkened skin. Dipyridamole is not known to act on ENT3 in follicles, so this is an analogy only. Molho-Pessach 2008

Nobody has tested whether topical dipyridamole reaches the dermal papilla or adds anything over adenosine itself.

What the studies show

  • No study of dipyridamole on hair, follicles or dermal papilla cells exists in any species, and no trial is registered (checked 2026-10-10).
  • A cosmetics-company trial of 0.75% topical adenosine in 30 Japanese women with female pattern hair loss beat placebo over 12 months on anagen rate and thick-hair rate. Oura 2008
  • In 102 Japanese men, adenosine beat a niacinamide lotion on the share of thick hairs at 6 months, again in a company-linked trial. Watanabe 2015
  • An adenosine-receptor agonist gave a sevenfold rise in follicle number at wound edges in diabetic mice. Mice only. Sun 1999
  • A 1% dipyridamole nanoemulgel penetrated skin in a rat psoriasis model, with no hair outcome. Maded 2024
  • The US label lists alopecia among post-marketing reports, with no frequency. US label

How it's used in studies

No hair dose exists. For orientation, oral dipyridamole is used at 75-100 mg four times daily alongside warfarin after valve replacement.

Side effects and safety

Oral use causes dizziness (13.6% vs 8.2% on placebo plus warfarin in the label data), abdominal distress, headache and rash; rarer low platelets, low blood pressure and gallstones. It is an antiplatelet and adds to bleeding risk with anticoagulants or aspirin. Topical use on the scalp has never been studied; it is bright yellow and stains.

Bottom line

Plausible mechanism, zero hair data. The tested, cosmetic version of the idea is topical adenosine, and the proven drug is minoxidil. Discuss with a doctor.

Fun facts

  • The effect that makes dipyridamole useful in cardiac stress tests, flooding tissue with adenosine, is the one hair-loss hobbyists hope would wake up follicles.
  • H syndrome, caused by losing a nucleoside transporter, makes people unusually hairy and darkly pigmented. Molho-Pessach 2008
