Statins (simvastatin, simvastatin plus ezetimibe)

A cholesterol-pill combination regrew hair in a few dramatic alopecia areata case reports, then mostly failed in the open studies that followed; nothing shows topical statins help ordinary balding.

hair loss

What it is

Simvastatin is a prescription cholesterol drug; ezetimibe blocks cholesterol absorption. The tablet combination is sold as Vytorin or Inegy. Hair use is off-label.

How it works

Follicles need balanced cholesterol production. Mice lacking Insig in skin pile up cholesterol precursors and fail to cycle hair; topical simvastatin lowered the precursors and corrected the defect. Evers 2010 Statins are also proposed to calm the immune system, the rationale for alopecia areata. In a lab study, simvastatin pushed follicle stem cells toward keratinocytes. Babakhani 2019

What the studies show

  • One person with alopecia universalis had dense scalp hair a month after adding ezetimibe 10 mg to simvastatin 40 mg; two years of simvastatin alone had done nothing. Robins 2007
  • Two more refractory cases benefited, with continued steroid injections. Ali 2010
  • In an open study of 14 people, 4 (28.6%) were responders with 30-80% regrowth after 3 months. Choi 2017
  • In 12 patients, 67% had no regrowth, 24% had muscle pain, and the authors judged it ineffective. Freitas Gouveia 2017
  • A phase 1 trial (NCT01520077, 29 people) was completed in 2015; we could not read its results. Several other letters gave no usable numbers.
  • No topical statin trial for pattern hair loss was found (2026-10-10). Recent topical ointments were used in rare genetic cholesterol-pathway disorders, with minimal reported hair outcome.

How it's used in studies

Oral simvastatin 40 mg plus ezetimibe 10 mg daily in the alopecia areata series. Topical forms in the genetic-disease reports were compounded ointments.

Side effects and safety

Muscle pain in 24% in one series. Statins are prescription drugs with liver and muscle risks and are avoided in pregnancy.

Bottom line

Conflicting, uncontrolled evidence for alopecia areata and nothing for pattern baldness. JAK inhibitors have far stronger evidence (see tofacitinib and ruxolitinib). Discuss with a doctor.

Fun facts

  • Hair regrew in some "incurable" alopecia universalis cases after a cholesterol pill, but when more patients were tested the effect mostly vanished.
