Anticoagulants (heparin, warfarin, DOACs)
Blood thinners that can cause shedding about 3 months after starting, which usually reverses and is never a reason to stop on your own.
What it is
Drugs that prevent or treat blood clots: heparins (injections), warfarin (tablets) and direct oral anticoagulants (rivaroxaban, apixaban, dabigatran, edoxaban). All are prescription-only. Stopping or switching one risks stroke or clots, so hair changes belong in a conversation with the prescriber.
How it works
The mechanism is uncertain. Timing and appearance point to telogen effluvium, and heparins and coumarins cause a similar pattern. Hypotheses include damage to the follicle's blood supply or an antimitotic effect of heparin. Watras 2016
What the studies show
- A review reports older figures of alopecia in 54% to 66% of people on unfractionated heparin and 30% to 40% on coumarins, usually starting after about 3 months. These are 1950s-era numbers that probably overstate modern rates. One unreviewed registry poster gave 4.4 per 100 patient-years for two DOACs. Watras 2016
- Among 1,316 DOAC reports in the WHO database, 80% were women, the median onset was 28 days and rivaroxaban made up 58.8%. Reports have no denominator, so they say nothing about incidence. Bonaldo 2020
- A drug-safety database study flagged alopecia as a signal for rivaroxaban. Guo 2026
- A 20-year-old with recurrent clots lost hair on several heparin derivatives and DOACs, quickly reversibly, but not on fondaparinux. Weyand 2017
- Warfarin-related alopecia resolved after a switch to apixaban in one dialysis patient. Hull 2021
- A review of 192 drugs placed enoxaparin among 13 with strong evidence of an alopecia link. Mounessa 2023
- A review called alopecia a common complication that can interfere with decisions to extend treatment, particularly in women. Marques 2020
- The US heparin and warfarin labels list delayed transient alopecia and alopecia respectively. Heparin label Warfarin label
The conditions being treated (clots, surgery, pregnancy events, cancer) are themselves shedding triggers, so causality is hard to separate. No trial tracked hair outcomes.
Side effects and safety
The main risk of these drugs is bleeding; hair loss is minor by comparison. Flag any new supplement to the anticoagulation clinic, since warfarin dosing is sensitive to vitamin K.
Bottom line
If you shed on an anticoagulant, check ferritin, thyroid and recent illness first (iron and ferritin), give it time (see telogen effluvium), and tell the prescriber if it distresses you. Do not self-stop. Discuss with a doctor.
Fun facts
- The effect can be agent-specific: one patient lost hair on heparins and DOACs but not on fondaparinux. Weyand 2017

- Beta-blockersHeart and blood-pressure drugs that rarely cause reversible shedding, based on old case reports only.
- Antithyroid drugs (carbimazole, methimazole, propylthiouracil)Medicines for an overactive thyroid; shedding on them can come from the thyroid disease, an over-treated thyroid, or the drug, which is hard to separate.
- Iron and ferritinLow iron stores are common in people who shed hair, but no trial has shown that iron pills regrow hair, and nobody agrees on a hair-specific ferritin target.
- Telogen effluviumA temporary burst of diffuse shedding that starts 2 to 3 months after a physical or emotional shock and usually settles within 6 months once the trigger is gone; it does not cause bald patches.
